Case Management


Our Traditional Case Management services focus on connecting participants with the essential resources they need for daily functioning and long-term stability.
Services include:

Assistance with housing applications

Medicaid and SSI/SSDI navigation

Referrals to community programs and services
Case Managers work one-on-one with participants to reduce barriers, advocate for services, and ensure follow-through with care.
Recovery Management Services (HCBS-AMH)

Our Recovery Management Services are offered through the Home and Community-Based Services – Adult Mental Health (HCBS-AMH) program, administered by the Texas Health and Human Services Commission (HHSC). This program is designed to support individuals with serious mental illness (SMI) who are at risk of institutionalization or have complex care needs that require a coordinated, community-based approach.
Eligibility Criteria:
To qualify for HCBS-AMH services, participants must meet HHSC-defined criteria, which generally include:
A diagnosis of serious mental illness (e.g., schizophrenia, schizoaffective disorder, bipolar disorder, major depressive disorder)

History of repeated psychiatric hospitalizations or extended stays in state hospitals

Recent or current residence in a nursing facility, residential treatment center, or similar institution

Demonstrated functional impairments that impact daily living, community integration, or housing stability
All eligibility determinations are made by HHSC.
Our Recovery Management team provides:

Individualized Recovery Planning (IRP): Collaboratively developed and reviewed regularly to reflect participant goals

Care Coordination: Seamless connection to behavioral health, medical, housing, and social support services

Monitoring & Advocacy: Ensuring all services align with participant needs and comply with Medicaid and HHSC standards

Transition Support: For individuals stepping down from institutional care back into the community

Wellness Checks & Follow-Up: Ongoing engagement to promote safety, progress, and stability
Our Recovery Managers ensure that participants receive the right services at the right time from mental health and medical care to housing and social supports while respecting each person’s goals, preferences, and cultural identity.
